We are accepting new patients under the age of 18.  For patients transferring care, it is helpful for us to have a copy of your child's immunization and medical records.  You can contact your child's previous pediatrician and request a release of medical records from them.   

Alternatively, you may sign our release of medical records form and we can contact your previous pediatrician's office directly.  Please download and fill out our New Patient Forms and bring them to your first appointment.

Call to schedule your first appointment 2-3 days after you are discharged from the hospital.  We will register your baby with the practice at that visit.  Please refer to "Are Y​ou Expecting?" for more information.
