Students participating in high school athletics programs are required to have a sports physical or pre-participation exam prior to starting their sport.

If your child has had a well child exam within the past year, we may be able to fill out your paperwork based on the most recent well child check. Please call our office to determine if an office visit is needed.

Each school district and private school has their own unique clearance form. Please fill out the patient history section prior to you appointment and bring that and the physician clearance form with you to the office.
